Lakeside Student Ministry Official Permission Slip

I/we give permission for my student to participate in the event named below sponsored by Student Ministries of Lakeside Baptist Church. I/we understand that there are inherent risks involved in any ministry trip or athletic event. I/we agree to release, covenant not to sue and forever discharge Lakeside Baptist Church and their leaders and volunteers from any and all claims which allege negligent acts and/or omissions related to any accident or injury incurred by my student while on this trip and from any legal responsibility related to the trip.

Also, I understand that should it become necessary for my student to come home early, as determined by the supervising adults, I will arrange for the immediate transportation of my student.
